Express Phleb respects your privacy. This policy explains what information I collect through expressphleb.com and how I use it.

 

Information you give me: When you use my appointment request form, I collect your name, your email address and/or phone number, your city or service location, your preferred appointment date, whether you have a provider's laboratory order (yes/no), and any short message you choose to include. I use this information only to respond to your appointment request and arrange your mobile blood draw. I do not sell your information, and I do not share it with third parties for marketing.

 

What not to send: Please do not send detailed medical histories, laboratory results, or other sensitive health records through the website contact form. It is an ordinary web form, not a secure medical portal. If your situation requires sharing sensitive information, I'll arrange an appropriate method when we speak. Note: Express Phleb collects blood specimens. Laboratory testing is performed by Labcorp, Quest Diagnostics, or the laboratory you choose — not by Express Phleb.

 

Calls, texts, and email: If you call, text, or email me at (970) 319-4183 or [email protected], I keep that correspondence only as long as needed to serve you.

 

Data retention: I keep appointment-request information only as long as necessary to provide my services and meet any legal record-keeping obligations, then delete it.

 

Changes: If this policy changes, I'll post the updated version here with a new effective date.
